Community Health Educator

Plans, markets, implements, and evaluates community education and outreach programs to groups in classroom and community settings on a variety of topics related to reproductive health. Must have a demonstrated ability to establish networking relationships in the community. Job requires commitment to quality healthcare and excellence in customer service. High energy needed.


Experience or training in the area of reproductive health, human sexuality, adolescent development, and learning theory preferred. Training in group dynamics helpful. Experience working with youth and diverse populations, speaking in public, and implementing, coordinating, and evaluating programs preferred. May occasionally require negotiating skills and use of discretion. Self-motivation, self-direction, teamwork, and organizational skills required.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Directly recruits patients and promotes PPGOH services in specific grant-funded counties.
  • Plans, markets, implements, and evaluates community health and education activities for the PPGOH service area.
  • Participates in planning, marketing, implementing, monitoring, and evaluating designated education programs, workshops, and conferences.
  • Writes and adapts lesson plans and curricula for specific programs and diverse populations.
  • Develops and presents information on family planning, STD/STI prevention and treatment, and promotes sexual health to adolescents and adults in school and community settings.
  • Plan and implement education and outreach initiatives that respond the cultural and linguistic characteristics of persons living in the PPGOH service area.
  • Visits outreach sites such as businesses, schools, churches and community service agencies to establish referral relationships, partnerships, and outreach activities.
  • Represents PPGOH at community meetings and coalitions as assigned.
  • Promotes PPGOH services at local health fairs and other community events.
  • Conducts STD/STI tests and processes samples for health center review and follow-up.
  • Serve as in-house resource for PPGOH staff training on issues of expertise.
  • Maintains current demographic, cultural, and epidemiological profile of the communities served by PPGOH.
  • Maintains accurate records of education and outreach activities.
  • Collects feedback from program participants on a regular basis.
  • Assists in the preparation of department goals, objectives, planning, evaluation, and reports.
  • Uses Community Leadership Councils as a resource in client base recruitment.
  • Attends regular departmental meetings and assists with department maintenance and other duties as assigned.



Job is not routinely supervisory in nature. May occasionally require recruitment and coordination of volunteers for education activities.

LEVEL TWO: Position requires a Bachelor’s Degree in Education, Social Service, Community Health, Communication, or related field. Minimum of 4 years of related work experience will be considered in lieu of a degree. Presentation and training skills required.



LEVEL THREE: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.



LEVEL ONE: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to interpret bar graphs.



LEVEL TWO: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.


Frequently requires creativity, problem-solving, decision-making, interpersonal skills, flexibility, and independent judgment. May occasionally require negotiation skills and use of discretion.




Requires evidence of valid Ohio driver’s license and automobile insurance in accordance with agency and liability policy.

While performing the duties of this job, the employee is regularly required to stand, walk, talk, and hear. The employee frequently is required to use hands to finger, handle, or feel and reach with hands and arms. The employee is occasionally required to sit. Work requires occasional bending and stooping. Rarely requires repetitive motion of the feet. Must be able to occasionally lift, push and/or carry up to 30 lbs., operate a computer including keyboard, and operate multi-key telephone. During peak activity periods work in excess of 7.5 hours per day and/or 37.5 hours per week.

Job involves frequent interruptions, occasional overtime, some evening, early morning, and weekend hours, and frequent travel throughout the PPGOH service area. Work is performed in an office environment as well as in a variety of community and school settings. The noise level in the work environment is generally moderate. Reliable transportation necessary.
